APACS Standard 40 host simulator

Initial version of APACS 30/40 host simulator added into iso8583.info hosts.
Few simple Authorisation flows were checked and can be approved for known cards.

Keyed, Swipe, and EMV Chip transactions can be processed and validated for supported tests.

Acquirer Specific data and Auxiliary Data records are not filled in the host simulator responses.
